The child care program is designed to provide affordable accident insurance for students of licensed child care centers, Head Start programs, and nursery schools.

This insurance program pays primary or excess benefits for usual and customary incurred expenses for necessary medical or surgical treatment, services, supplies, and expenses for injury to natural teeth.  Covered expenses are paid if incurred within 52 weeks of the date of the accident.

Accident Medical Expense Benefits

Benefits of the Insured may include:

• treatment by a physician
• hospital services
• services of a licensed practical nurse or RN
• radiology services
• use of operating room, anesthesia (including the administration thereof)
• laboratory services
• use of an ambulance
• ambulatory medical center or surgical center
• prescription medicines if ordered by attending physician
• therapeutic service or supplies
• home health care expenses

Accidental Death and Dismemberment (AD&D) Benefits:

In the event of a serious injury which results in the death or dismemberment of a child, benefits will be paid in accordance with the policy schedule.  The principal maximum benefit is $5,000.
